SDSU Mentoring Terms & Conditions
STUDENT RELEASE OF LIABILITY, PROMISE NOT TO SUE, ASSUMPTION OF RISK AND AGREEMENT TO PAY CLAIMS

Activity: Aztec Mentor Program, including Res-AMP and all other tracks
Activity Dates: Fall and Spring semesters
Activity Locations: Greater Southern California region (in person), nationally and internationally (virtual)
In consideration for being allowed to participate in this Activity, on behalf of myself and my next of kin, heirs and representatives, I release from all liability and promise not to sue the State of California, the Trustees of The California State University, California State University, San Diego State University, and their employees, officers, directors, volunteers and agents 
(collectively “University”) from any and all claims, including claims of the University’s negligence, resulting in any physical or psychological injury (including paralysis and death), illness, damages, or economic or emotional loss I may suffer because of my participation in this Activity, including travel to, from and during the Activity.   
I am voluntarily participating in this Activity.  I am aware of the risks associated with traveling to/from and participating in this Activity, which include but are not limited to physical or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent disability 
(including paralysis), economic or emotional loss, and/or death.  I understand that these injuries or outcomes may arise from my own or other’s actions, inaction, or negligence; conditions related to travel; or the condition of the Activity location(s).  Nonetheless, I assume all related risks, both known or unknown to me, of my participation in this Activity, including travel to, from and during the Activity.  
I agree to hold the University harmless from any and all claims, including attorney’s fees or damage to my personal property, that may occur as a result of my participation in this Activity, including travel to, from and during the Activity.  If the University incurs any of these types of expenses, I agree to reimburse the University.  If I need medical treatment, I agree to be financially responsible for any costs incurred as a result of such treatment.  I am aware and understand that I should carry my own health insurance.  
I am 18 years or older.  I understand the legal consequences of signing this document, including (a) releasing the University from all liability, (b) promising not to sue the University, (c) and assuming all risks of participating in this Activity, including travel to, from and during the Activity. 
I understand that this document is written to be as broad and inclusive as legally permitted by the State of California.  I agree that if any portion is held invalid or unenforceable, I will continue to be bound by the remaining terms.   
I have read this document, and I am agreeing to it freely.  No other representations concerning the legal effect of this document have been made to me.

MENTOR CODE OF CONDUCT 


Thank you for your interest in becoming an Aztec Mentor Program mentor. Before you are paired with an SDSU student mentee, we ask every mentor to acknowledge the commitment necessary to ensure a successful mentoring partnership. Participation in the Aztec Mentor Program is highly enjoyable, rewarding and beneficial for both alumni and students, which requires mutual respect and commitment from the participants. 

Please know, the university is committed to maintaining a safe and healthy living and learning environment for students, faculty and staff. Each member of the campus community should choose behaviors that contribute toward this end. Students are expected to be good citizens and to engage in responsible behaviors that reflect well upon their university, to be civil to one another and to others in the campus community, and contribute positively to student and university life. (41301, Title 5, California Code of Regulations) 

IMPORTANT: As a registered mentor, I acknowledge and understand the following:

I pledge to commit my time and expertise to assist my mentee in developing and achieving his or her goals. 

I commit to building a mutual relationship with my mentee, to hearing about dreams and challenges, to sharing my own stories, and to respecting the differences between us. 

I understand that students request mentors based on educational backgrounds, professional interests, and other factors, but a “perfect” pairing is not always possible. 

I understand the quality of any mentoring experience is largely determined by the commitment, time and quality of communication between me and my mentee. 

I understand that my mentee is responsible for transportation to and from any meeting. 

I understand and support that alcohol should not be part of AMP. I further agree to abide by these guidelines as they relate to any mind-altering substances. 

I am aware of SDSU's policy relating to nondiscrimination and unlawful harassment and I agree to abide by university policy with regard to my interaction with my mentee.

If I become aware of an incident that may violate university policy, I will immediately inform Dr. James Tarbox, Executive Director of Career Services, or Diane Marin, AMP Coordinator, at amp@sdsu.edu.

I understand that my involvement in the Aztec Mentor Program is at the discretion of SDSU Career Services and Career Services reserves the right to withdraw me from the program if I do not meet program expectations. 

If at any point I am no longer able or no longer want to participate in the program, I promise to notify my mentee and Career Services (amp@sdsu.edu) about my situation as soon as possible so a new mentor can be matched for the mentee.

I understand that the program runs during the Fall and Spring semesters and that there is no required commitment after the program ends. However, if both parties choose to continue a professional relationship SDSU Career Services is not liable for this interaction. 

I give SDSU Career Services permission to share my name, e-mail address and phone number with my mentee.

I acknowledge all of the above statements and by registering for this program I commit to mentoring a current SDSU student in the Aztec Mentor Program. This Code of Conduct will remain in effect until I request to opt out via email to amp@sdsu.edu

Chronus Terms of Service
Acceptable Use Policy
This Acceptable Use Policy (“AUP”) applies to your use of the Chronus Services (“Services”) and any content you contribute (“Content”). You agree you will not use the Services for any purpose that is unlawful or prohibited by this AUP. You may not use the Services in any manner that could damage, disable, overburden, or impair the Services, networks connected to the Services or interfere with any other party's use and enjoyment of any Services. You may not attempt to gain unauthorized access to any Services, other accounts, computer systems or networks through any other means. You may not obtain or attempt to obtain any materials or information through any means not intentionally made available through the Services.
User Conduct. The Services allow you to submit Content. You agree to only submit Content that is proper. By way of example, and not as a limitation, you agree that when submitting Content, you will not:
  • Defame, abuse, harass, stalk, threaten or otherwise violate the legal rights (such as rights of privacy and publicity) of others.
  • Publish, post, upload, distribute or disseminate any inappropriate, profane, defamatory, obscene, indecent or unlawful topic, name, material or information.
  • Submit Content in connection with surveys, contests, pyramid schemes, chain letters, junk email, spamming or any duplicative or unsolicited messages (commercial or otherwise).
  • Upload, or otherwise make available, files that contain images, photographs, software or other material protected by intellectual property laws, including, by way of example, and not as limitation, copyright or trademark laws (or by rights of privacy or publicity) unless you own or control the rights thereto or have received all necessary consent to do the same.
  • Upload files that contain viruses, Trojan horses, worms, time bombs, cancelbots, corrupted files, or any other similar software or programs that may damage the operation of another's computer or property of another.
  • Download any file posted by another user of the Services that you know, or reasonably should know, cannot be legally reproduced, displayed, performed, and/or distributed in such manner.
  • Advertise or offer to sell or buy any goods or services for any business purpose, unless such Submission Tool specifically allows such messages.
  • Violate any applicable laws or regulations.
  • Create a false identity for the purpose of misleading others.
Chronus has no obligation to monitor your Content. However, we reserve the right to review materials posted and to remove any materials in our sole discretion.
Chronus reserves the right to terminate your access to any or all of the Services at any time, without notice, for any reason whatsoever.
Chronus reserves the right at all times to disclose any information as we deem necessary to satisfy any applicable law, regulation, legal process or governmental request, or to edit, refuse to post or to remove any information or materials, in whole or in part, in our sole discretion.
Use caution when giving out any personally identifiable information. Chronus does not control or endorse the content, messages or information submitted in the Services and, therefore, Chronus specifically disclaims any liability with regard to any submissions in the Services and any actions resulting from your submission of Content and participation in any areas where content may be submitted in the Services.
Use of Content. Your Content will be able to be viewed by other users of the Services. You should only provide your Content if you are comfortable sharing it with others.
You understand that Chronus does not guarantee the confidentiality of your Content and you shall be solely responsible for your Content and the consequences of submitting and publishing your Content on the Services. You represent, and warrant that you own or have the necessary licenses, rights, consents, and permissions to publish your Content.
You retain all right in your Content. By submitting your Content through the Services you hereby grant Chronus a worldwide, non-exclusive, royalty-free, sublicenseable and transferable license to use, reproduce, distribute, prepare derivative works of, display, and perform your Content in connection with the Services.
Feedback. Any suggestions, comments, or other input you give to Chronus (“Feedback”), even if designated as confidential, will not create any confidentiality obligation for Chronus. Chronus will be free to use, reproduce, license or otherwise distribute, and exploit Feedback to improve and enhance Chronus products, technology, services, documentation or otherwise. Chronus’ use of Feedback does not create any obligation to you. You agree not to provide Chronus with any Feedback that is subject to license terms that seek to require any Chronus products, technologies, services or documentation incorporating or derived from such Feedback, or any of Chronus intellectual property, to be licensed or otherwise shared with any third party.
